I have an arcade controller with Ultimarc sticks (gamepad) and buttons that send keyboard presses, as well as buttons for pinball on the sides.  Everything works flawlessly in MAME, but PinballX will not recognize the joystick presses for mapping.  It would be extremely convenient if I could use the joysticks on my controller for navigation in PinballX.

Sorry, might be a dumb thing to say but are you sure it is not acting as a keyboard? If not are you configuring under the right section? Keyboard for keyboard, Joystick for Joystick.

Again, sorry if its a dumb thing to say but I cant think of any reason why a gamepad device would not work, what exactly happens?

It's an X-Arcade controller with Ultrastik joysticks... the Ultrastiks are gamepad, the buttons detect as key presses... so it is both.  Works perfectly in MAME.  If I go into the configuration screen (either one) the joystick presses are not detected so I cannot assign them.

Pinball X doesn't respond to "analogue" joystick functions as for as I know. So that "may" be the problem with the UltraStick itself.  

If the UltraSticks are instead configured to use d-pad (also known as HAT switch) functions for the stick, then Pinball X won't recognise that either.... at least it doesn't recognise d-pad controls from an xbox 360 gamepad like on my cabinet.

Isn't that where Xpadder, JoyToKey, and X360ce programs step in to convert analogue joystick commands to keyboard output?

They can, but sometimes you may have an issue when you launch a game from Pinball X, and the X-padder like software is still thinking you are "playing" from Pinball X and will still run the keyboard mapping intended for Pinball X. This means that his analogue functions and the "mapped" functions (on his stick) are running at the same time.
