I am running MAME32 v.96u1 with an IPAC. When I launch using the menu system from the Tiptonware compilation, it works fine in every game. When I use GameEx, the button (mapped to L-Cntrl) stops working. All other buttons work fine. I have tried reprogramming it to a letter key (H), and I have the same problem. It doesn't matter what I change it to; it works unless I use GameEx. I have wasted literally hours trying to figure it out. It is therefore something really stupid. Any ideas? The specific game I like to play the most is affected (Block Out). I can't say if it happens in all games. I am one button away from being able to enjoy GameEx.

Gameex wont block your keys. Did youre-config MAME after changing your IPAC button 1?

Yes. I just learned that it only seems to do it in BlockOut. I tried a few other games and they work fine. I notice that when I press tab to change the mappings for the game, I get shown a slightly different set of info than when I do it after having launched through GameEx. Specifically, the order of the buttons on the mapping screen are different. This challenges my understanding of how things work (which isn't hard to do.) Unfortunately, I love the game so much that I can't let it go. Is there a path to a config file that isn't set correctly, and GameEx is trying to use a different one? Sorry, I'm very new at this. I was able to reprogram keys so that I can use my buttons to navigate the features of GameEx like adding backspace.

What's your commandline? Maybe you're not calling your controls correctly.

I'm quite embarrassed to say I don't know. I have only used GUI to start games. How can I see what GameEx is doing before it starts a game?

I have enabled IPAC support in the input settings. In the logfile, it says: -triplebuffer -nowindow -joy -skip_gameinfo

Well can use Advanced Configuration or Setup Wizard in the mame settings. However you pointed to your directories when you set up mame. Check the commandline settings and copy and paste it here so I can have a look see.

I am not using advancedMAME. In fact, I'm not using any other emulators. I don't know if this sheds any light on it, but it also tries to add coins when the game first launches. Don't know if this is a feature or a symptom.

If you look in your Mame folder, there is another folder called "cfg". in that folder is default.cfg, which is your standard setup for player1, 2, etc. There should be a file in there called Blockout.cfg (or whatever the romname for blockout is) that file would contain any mappings that you have setup for blockout. Move that file out of the cfg folder, or edit the controls for blockout in mame, and It should fix the problem.

The mystery has been solved...and I am the stupidest person that has ever built their own cabinet. This was the problem:

There are three versions of Blockout in my roms; only one version 's keys work properly. GameEx sorts them differently, and I wasn't paying attention to which game I was selecting. Outside of GameEx, I chose the topmost version of the game (which you get trained to do) and it worked. You basically don't try the versions below it. As I have not cleaned up my roms yet, GameEx was sorting a different version to the top and I just picked it by rote. Thanks everyone for trying to help.
