I have the same problem. Hope someone has a answer. :)

When that happened to me, I went nuts trying to figure it out. I accidentally stumbled across the solution. For me the solution was in the Advanced Mame Settings in the configuration GUI. The third option down is "Bypass Disclaimer/Warnings." It was set to "yes." I changed it to "no." After that, the games loaded up. I hope this is the solution to your problem!

Still no luck for me

Does anyone have any other suggestions

Any ideas much appreciated

First thing to try is mame from the command line to make sure you can start it from a dos box.

Or use a MAME32 build, like MAME32FX for example. If it can run your games fine, hook it up to GameEx and that should work fine as well.
