How do you use this outside GameEx?

It sounds like it is a replacement for the rankings.ini file and I do not believe GameEx supports that file either.

That's just a favorites file. You would use it the same way you'd use your own personal favorites.ini and there's a setting under MAME in GameEx for this. You can also use custom lists, generally the same idea:

My concern with using lists like this is that it is based on someone else's opinion of what is good or bad.  But that is just my opinion. 

It is curious to see what others think is good or bad.
